Immunizations

It is state law that all students have the following immunizations on file in the school that they attend.

DPT/DT/TD - At least five doses required, one dose after 4th birthday, then one dose every ten years.
OPV - At least four doses, unless 3rd was given after 4th birthday.
MEASLES - 2 doses by age 3 or entry to Kindergarten
MUMPS - One dose after 1st birthday.
RUBELLA - One dose after 1st birthday.
HEPATITIS B - 2004-2005 K - 5th & 7 - 10th three doses required
2005-06 K - 11 three doses required
2006 -07 K-12 three doses required
VARIVAX (Chickenpox) - One dose of vaccine on or after 1st birthday
2004-05 K-4 & 7- 10
2005-06 K - 5 & 7-10
2006 - 07 k - 12 ( 2 doses if given at 13yr or older)
